If you answer YES to any of the following questions, please call the office at (937) 885-4412 to reschedule your appointment.

1. In the past 14 days, have you experienced any symptoms of Covid-19 including:

   • fever or chills

   • cough

   • shortness of breath or difficulty breathing

   • fatigue

   • muscle or body aches

   • headache

   • new loss of taste or smell

   • sore throat

   • congestion or runny nose

   • nausea or vomiting

   • diarrhea

2. In the past 14 days, have you been in close contact with anyone experiencing symptoms or who has recently tested positive for Covid-19?

3. Are you isolating or quarantining because you may have been exposed to a person with Covid-19 or are worried that you may be sick with Covid-19?

4. Are you currently waiting on the results of a Covid-19 test?
